Liability Waiver and Participation Acknowledgement and Release for Elsberry Flag Football Organization.

• I, the undersigned, wish to participate in any and all activities related to the Elsberry Flag Football Organization (the "Organization"). I fully understand and acknowledge that my participation in these activities may involve physical and mental risks, including but not limited to injury, discomfort, or other health-related issues.

• By signing this waiver, I hereby agree to the following:

• Assumption of Risk: I understand and accept the risks associated with my participation in the activities organized by the Elsberry Flag Football Organization. These activities may involve physical exertion and mental stress, and I acknowledge that injuries or conditions, both physical and mental, may result from my participation.

• Release of Liability: I, on behalf of myself, my heirs, legal representatives, and assigns, hereby release and hold harmless the Elsberry Flag Football Organization, its officers, employees, agents, volunteers, and any affiliated persons or entities from any liability, claims, demands, or causes of action that may arise from my participation in the activities, including injuries or damages, whether caused by negligence or otherwise.

• No Liability of Facility Owners: I further understand and agree that the owners or operators of the facilities where the activities take place shall not be held liable for any injuries, accidents, or other incidents that may occur during my participation in the activities.

• Health and Safety: I acknowledge that it is my responsibility to consult with a physician or other healthcare provider if I have any concerns regarding my health or fitness for participating in the activities. I also agree to follow all rules, guidelines, and instructions set forth by the Elsberry Flag Football Organization for my safety and the safety of others.

• Voluntary Participation: I affirm that my participation is voluntary, and I fully understand the potential risks involved. I am under no obligation to participate and may choose to withdraw from the activities at any time.

• Emergency Medical Treatment: In the event of an emergency, if I require medical treatment, I consent to the administration of first aid, emergency medical care, and transportation to a medical facility if necessary. NOTE: If you have any known medical concerns please advise the league by emailing Info@ElsberryFlagFootball.com and notify your coach.

• Code of Conduct: In the event that You, Your Player or Anyone Affiliated are deemed negligent of maintaining a professional code of conduct you may be ejected from the League or Tournament with no refund.

I, the undersigned, acknowledge that I have read and understood all of the information above. I consent to the terms outlined regarding injury response, medical concerns, and payment. I understand by electronically signing this waiver it is the same as hand written acknowledgement.
